TUITION: I understand that I am enrolling for the 2025-2026 dance year and that I am responsible for all tuition payments as registered.

WITHDRAWAL POLICY: I understand that a 30-day written notice is required to withdraw from the program.

CLASS PLACEMENT: I understand that class placement is at the discretion of the teachers. For all levels, Tap and Ballet are mandatory. Recreational dancers -- Raydance encourages dancers to add jazz and/or hip-hop to their training. Competitive dancers will be trained in all genres.

PERMISSION TO USE DANCER'S IMAGE: Raydance Entertainment Academy is granted permission to use the dancer's image in advertisements, news releases, social media, and on its website.

SICK POLICY: I understand that for the health and safety of all students, staff, and families, Raydance Entertainment Academy maintains a strict sick policy. I understand that I (or my child) will not atttend class if I have any contagious illness, including but not limited to flu, strep throat, pink eye, COVID, etc). I understand that adherence to this policy is essential for the well-being of the studio community and that make-up classes may be available in cases of illness-related absences.

ONLINE CLASS POLICY: I understand that if it should become necessary to close the studio due to State-Wide Mandate, due to health conditions (such as COVID or any other communicable diseases), classes will be switched to online. I further understand that there is a 30-day drop policy, and a completed Change Form is also required to drop online classes.

All students must complete and sign this form before taking classes at Raydance Entertainment Academy. If the student is under age 18, a parent or guardian must also sign this form.

I recognize and understand that the activity of dance and dance training may involve physically demanding activities, which could result in injury. I hereby release Raydance Entertainment Academy LLC, along with its dance teacher and employees, from all liability for any personal injuries contracted by my child or me while attending or participating in any dance classes, workshops, activities, or events of the Academy.

Please notify Raydance Entertainment Academy and instructors in writing of any physical limitations your child may have. If you are uncertain about your child's physical capabilities, the Academy recommends consulting a physician before permitting participation. Additionally, Raydance Entertainment Academy is not responsible for any lost, stolen, or damaged property.

I understand and agree that it is my sole responsibility to safeguard my personal property while attending or participating in any classes, events, or workshops. I hereby release Raydance Entertainment Academy, employees, and dance teachers from all liability for loss or damage to my personal property while attending or participating in classes, events, or workshops.

In the event of physical injury, I understand that Raydance Entertainment Academy will notify the named emergency contact(s) of my condition and how to reach me. In the event of an extreme medical emergency, I hereby authorize Raydance Entertainment Academy to notify the named emergency contacts as well as to have my child transported to a medical treatment facility as necessary.

Raydance Entertainment Academy reserves the right to use photographs of students participating in class, events, or performances to advertise and promote the Academy. I understand that no financial compensation will be given for the use of these photographs/videos.
